Sep-2014 : 62 years old lady had subtrochanteric fracture for which IM Nail was done. This failed within 1 month of surgery for which fixation with DCS was done along with bone grafting. Post operative period was uneventful but 5 months after this surgery the DCS broke with clear evidence of non union of the fracture. For this difficult case this plate was removed and it was decided to convert this into uncemented long stem metal on poly Total Hip Replacement. Following surgery the patient is comfortable and mobilizing with the help of walker.

This patient, a 44 years old gentleman from Bangladesh, has presented to us with deformity at right hip with shortening of right lower limb. He had a history of road traffic accident in 2012 and was managed primarily in Bangladesh. He had a fracture dislocation of right hip which was missed initially by the primary surgeon. The fresh radiograph showed dislocated right hip with changes suggestive of AVN of femoral head. CT scan of pelvis with 3D reconstruction was done and revealed posterior acetabular wall defect with dislocated hip. Now he has undergone right uncemented Total Hip Replacement with reconstruction of acetabular defect using bone graft fixed with screws and trabecular metal continuum shell. His deformity and shortening of right lower limb has been corrected. He has been discharged and walking comfortably and painfree without any support.
June-2014: 67 years old gentleman presented to us with complains of pain around right hip with difficulty in walking. He was operated elsewhere for intertrochanteric fracture femur of right side 10 yrs back (ORIF with Dynamic Hip screw) and for fracture neck of femur of left side 3 yrs back( Bipolar Hemiarthroplasty). On examination he has painful and decreased ROM at right hip with shortening of right limb. Fresh radio-graph showed changes suggestive of AVN of right femoral head with ununited fracture neck of femur and superolateral cut out of DHS screw. He has undergone removal of DHS and conversion to Uncemented Total Hip Replacement using fully porous coated long femoral stem. He had regained back his movement at right hip which are pain free and the previous shortening has also been corrected. He has now discharged and is walking comfortably without any support.
May-2014 : Revision of Metal on Metal Total Hip Replacement due to Pseudotumor formation 39 years old gentleman is a follow up case of right total hip replacement (Large Diameter head Metal on Metal) done 5 years back for healed tuberculosis of hip. Now patient has presented with complains of pain around right hip on walking. Fresh x-rays showed radiolysis around acetabulum with no loosening of femoral stem. MRI of pelvis showed pseudotumor formation around right hip. He has undergone revision Right Total Hip Replacement using uncemented acetabulum and femur (Metal on Poly). Intraoperatively there was a change in the consistency of muscles around the hip joint (pseudotumor), this mass was excised in toto. Now the patient has been discharged and is now comfortable and pain free. This rare condition of pseudotumor formation with Metal on Metal Total Hip Replacement(THR) has been successfully treated.
Bilateral Total Hip Replacement 31 years old lady presented to us with pain in both hips and difficulty in walking. She is a known case of systemic lupus erythematous ( SLE) with avascular necrosis of both hips. On examination she had gross restriction of movements of both hips.
She had undergone uncemented Total Hip Replacement for both hips. She has been discharged and now walking comfortably and pain free.
